AREAS OF RESPONSIBILITY:
The Geriatric Care Manager is responsible for managing and providing Care Management services to clients. Care management services include meeting with prospective clients and providing a professional assessment and plan of care. The Care Manager may also assist with implementation of the care plan and provide ongoing monitoring to ensure quality, appropriateness and cost of effective services to clients. They will also perform other duties as assigned consistent with the skills and training and the goals and mission of Pathways.

QUALIFICATIONS:
Degree in social work, nursing, or gerontology with master’s level preferred.
Experience in geriatrics and/or fee for service care management required; other professional experience may substitute. Care Management Certification (CCM or CMC) a plus.
Experience and demonstrated competence and comfort working with families in crisis, the frail elderly, the disabled and/or the terminally ill.
Valid California driver’s license and proof of current automobile insurance.
Excellent written and verbal communication skills required.
