Renewal Care Partners is seeking a dynamic and strategic Care Management Director to lead our mission-driven Client Services Team. Reporting to our COO/CFO, the Care Management Director is responsible for developing and supporting Care Managers and Care Partners in both New York and Chicago. Together, the Client Services Team provides innovative, holistic, person-centered homecare for all of our Clients.
Duties and Responsibilities
Oversees Care Managers’ portfolios of Clients and Care Partners to help maintain maximum satisfaction.
Leads Client Services team in coordinating effective plans, in response to Client/Client stakeholder feedback to improve Client satisfaction.
Ensures all services are provided in full compliance with state level health departments.
Manages the admission of new Clients and the ongoing experience of existing Clients.
Supports the recruitment, training, and retention of Care Partners.
Optimizes Care Managers’ use of our homecare software, to improve matching, scheduling, communication, and documentation related to both Care Partners and Clients.
Tracks data and utilizes analytics to inform decision-making, problem-solving, and goal setting as they relate to CS team.
Supports participation in research initiatives with leading academic institutions.
Develops relationships with schools of social work and oversee social work internship program.
Collaborates with other departments to foster diversity, equity, inclusion, and belonging.
Education & Experience
College degree is required; preference for candidates with graduate degrees in health or human services-related fields.
Homecare industry experience.
Knowledge of Alzheimer’s and dementia-related care.
Experience leading teams and managing people.
