Renewal Memory Partners is seeking a Care Coordinator to support our Client Services Team. Reporting to the Care Manager, the Care Coordinator is responsible for managing the schedules of Care Partners in Illinois and New York, consistent with each Client’s Plan of Care.
Duties and Responsibilities:
~ Ensures that case assignment, case management, and services provided are appropriate, adequate, and consistent with a Client’s Plan of Care and medical orders.
~ Establishes trust-based relationships with Clients, serving as their primary point of contact for day-to-day scheduling logistics-related concerns.
~ Serves as primary point of contact for day-to-day scheduling logistics concerns.
~ Maintains accurate Care Partner data, including restrictions, exclusions, preferences, and scheduling information.
~ Maintains open, constructive lines of communication with Clients, Care Partners, and other RMP staff.
~ Participates in the Recruitment process as needed.
Performs other duties as assigned.
Education and Experience:
~ 1+ years of Care Coordination experience preferred.
~ High school diploma required; Bachelor’s degree preferred.
~ Well-organized, self-motivated and positive attitude.
~ Proactive, solution-oriented team player.
~ Strong computer skills including MS Office (Word, Excel, PowerPoint & Outlook) and a strong knowledge of the
Internet (search tools, social networking tools, etc.).
