The Program and Outreach Coordinator is a combined position which oversees and coordinates all aspects of Lifespans Well Being Program, and conducts Outreach in our community and throughout our industry to promote Lifespan services.. Please see the details involved with that below.

Education and Experience:
1. Bachelor’s Degree or MSW preferred in Social Work or related field.
2. A minimum of 3 to 5 years’ experience working with seniors.
3. A minimum of 2 years of experience as a project manager or in a related field.
Additional Experience for Outreach Coordinator:
1. Experience in marketing and business development, preferably in the health-care industry and preferred in aging care.
2. Experience in public speaking and event coordination.
3. Experience in developing and maintaining community partnerships and relationships.
4. Experience in Web design and publishing.

Additional Desired Experience, Skills/Abilities for Well-Being Program Coordinator:
1. Experience in care or case management.
2. Experience in staff supervision.
3. Knowledge of local elder care activity resources
4. Strong leadership skills: adept at providing ongoing quality of life activities, trainings, and support to Activity Coordinators.
5. MSW, BSW, RN, MFT, MA in Gerontology, Occupational Therapy, Physical Therapy, or Recreational Therapy (highly desired)
Physical Requirements (both positions):
1. Ability to communicate clearly in direct conversations and by telephone on a daily basis.
2. Sitting/standing at a desk approximately 70% of time.
3. Occasional walking to, reaching and/or bending to access files.
4. Lifting of office supplies, not to exceed 25 pounds.
5. Ability to travel to/from and work in the Lifespan office as needed.
6. Ability to work from a home office.
Job Summary Well-Being Program Coordinator:
Coordinates Well-Being Program related components including marketing memberships, training and tracking of Activity Coordinators, Quality-of-Life assessments, assigning caseloads, managing schedules, and assisting in WB financials.

Supervisory Responsibilities:
1. Give verbal and written warnings to Activity Coordinators if appropriate, or gather information from involved parties and present to management.
2. Provide written record of warning to employee and indicate that a copy will be placed in the employee’s file.
3. Counsel Activity Coordinators when appropriate, regarding job performance, stress reduction and employment issues.
4. Collaborates with Care Manager and/or management to resolve client and/or employee issues.
5. Assist with orienting new Activity Coordinators.
6. Assist with training Activity Coordinators.
Duties/Responsibilities:
1. Participates in a prescribed orientation and in-service process which results in a knowledge and skill level necessary for role fulfillment.
a) Attends ongoing in-service trainings to enhance ad improve skill set.
2. Provides for the safety needs of clients, staff, and public.
a) Notifies appropriate staff person of an unsafe area and modifies unsafe area if feasible.
b) Communicates and/or consults with Staffing Manager to solve environmental and/or safety problems.
c) Participates in annual in-service programs related to safety and infection control.
d) Complies with agency policies regarding safety and infection control.
3. Duties related to Well-Being Program:
a) Participates in the process of hiring Activity Coordinators.
b) Manages training, including software.
c) Arrange for new Activity Coordinators to orient at the client’s if needed.
d) Manages new memberships and schedules/performs Quality of Life Assessments and reassessments.
e) Lead creation and implementation of a variety of therapeutic activities.
f) Markets to new and existing referral networks, maintains relations and community outreach.
g) Schedules bi-monthly staff meetings with Activity Coordinators.
h) Review of Well-Being membership hours billed (weekly).
i) Review of Activity Coordinators payroll (2x monthly).
j) Review of Activity Coordinators software documentation (weekly).
4. Duties related directly to clients:
a) Provides information regarding services to potential clients.
b) Solves client/Activity Coordinators issues in conjunction with Operations Team.
c) Notifies client/family of all changes in scheduling.
5. Markets Lifespan’s services to health professionals, care facilities and in the community.
Job Summary Outreach Coordinator:
Responsible for implementation of marketing and business development strategies to increase exposure to the public and professional community to grow Lifespan. Maintain relationships with current referral sources and develop new referral sources through outreach and public engagement. Conducts continuous review of results, initiating corrective action, and recognizing accomplishments.

Duties/Responsibilities:

1. Community relations, marketing, and maintaining/growing the reputation of the agency in the community.
2. Duties related to implementing the business development plan:
a) Works with, CEO/President, COO, and Community Liaison to develop and implement strategic marketing plan and annual marketing budget.
b) Designs and/or works with graphic designer for marketing materials including blog posts and photo imagery.
c) Makes presentations regarding Lifespan and aging related topics in the community.
d) Contributes to Lifespan newsletter targeted at clients, their families, referral sources and community partners on issues regarding eldercare, new Lifespan programs, and recent achievements.
3. Duties related to developing and implementing a marketing plan:
a) Works with, CEO/President, COO, and Community Liaison to develop and implement strategic marketing plan and annual marketing budget.
b) Designs and/or works with graphic designer for marketing materials including blog posts and photo imagery.
c) Distributes promotional material to libraries, senior centers, hospitals, residential communities, doctors’ offices, not-for-profit centers and maintains mailing lists.
d) Makes presentations regarding Lifespan and aging related topics in the community.
e) Contributes to Lifespan newsletter targeted at clients, their families, referral sources and community partners on issues regarding eldercare, new Lifespan programs, and recent achievements.
f) Serves as lead organizer for Lifespan on-site and off-site events.
g) Represents Lifespan at business and health fairs.
h) Maintains marketing calendar.
i) In collaboration with Community Liaison, designs and places advertising for print media.
j) Is involved and an active member of the community.
4. Duties related to External Marketing:
a) Responsible for outbound marketing to clients and their families, including print-based, web-based marketing materials, and blog development.
b) Prepares and presents Lifespan programs to the community based professional and not-for-profit groups that could benefit from Lifespan services.
c) Participates in professional and non-profit groups to generate leads.
d) Develops and delivers presentations, including trainings, to organizations and public groups on topics related to aging and life enhancement for elders.
e) Utilizes social media as a means to expand reach.
6. Duties related to Data Management:
a) Develops and maintains a contact email database of former clients and potential clients and their families.
7. Duties related to Office Support:
a) Explains Lifespan’s services and charges to potential clients and their family members.
b) Arranges for brochure/marketing materials to be sent when appropriate.
c) Works well as a member of the Lifespan team.
d) Attends and contributes to regular staff meetings.
