My Care Manager is looking for a Care Manager (Social Worker/MSW, RN, OT, PT or related field) to be a part of our team. Our Care Managers assist individuals and families in improving their independence, level of functioning and quality of life by coordinating an array of social and medical services. Key responsibilities include:
Independently refer for and coordinate needed services
Accompany clients to various medical and social appointments
Advocate to meet client’s needs
Consult and collaborate with other professionals and service providers to assure comprehensive care
Counsel and support clients families and significant others to maximize their well-being
Monitor the ongoing needs of clients through in-person meetings
Compassionately respond to emergencies and crisises
A passion of working with the geriatric population must be evident in all interactions. MSW, RN, PT, OT or related degree required; Three to five years’ experience with geriatric and special populations preferred. Certification required within 6 months of employment.
