ORGANIZATIONAL RELATIONSHIPS:
•Receives direction from the Vice President of Pathways Private Duty.
•Serves as Liaison between staff of Pathways Home Health and Hospice and Pathways Private Duty clients, field and office staff.
•Participates as a member on inter-company committees.

AREAS OF RESPONSIBILITY:
Management

•Manage the development and ongoing program needs of care management service line for Pathways.
•Manage, develop and maintain operational processes, which includes upgrade of methods for documentation of client service and billing systems for Care Management.
•Create marketing plans, oversee development of marketing materials, and participate in direct marketing to inform the community of this service line.
•Promote Pathways and general eldercare services and education to the community via special events, conference and community presentations.
•Supervise and coach the Care Manager’s services on their case load and client needs.
•Educate Pathways staff, both in Private duty and Home Health and Hospice on care management services and how to identify potential clients.
•Assure the provision of safe, high-quality services to Pathways Private Duty’s clients.

Care Management

•Provide a continuum of direct care management services to clients from start to end of service.
o Work with a geriatric population, their families and caregivers to provide care management services with the goal of supporting independence and the ability to stay safely at home preventing premature institutionalization.
o Conduct comprehensive assessment to determine client needs and assist in development of plan of care.
o Provide a range of options and obtain and coordinate services chosen.
o Serve as a liaison between medical, psychiatric, social service and community agency providers and individual clients.
o Monitor quality and effectiveness of services provided to client making changes to care plan as needed.
o Maintain knowledge of and working relationship with community resources.
o May provide social work support to stabilize Pathways home care clients as need is identified.
o Provide consultation, counseling and education to clients, caregivers and family members.
o Ensures clients complete the agency’s required forms along with an explanation of services provided by the Agency.
• Facilitate caregiver support groups and provide caregiver training on an individual and/or group basis when needed.
• Provide after hours advice as needed to the after-hours coordinator.
• Act as Liaison between Home Health and Hospice and Private Duty to build better working relationships and improve quality of care.

Private Duty Organizational Support

•Identify needs for change in administrative policies and client care policies, procedures and protocols.
•Participate in the development, maintenance and implementation of administrative and client care policies and procedures.
•Support teamwork and problem solving between the office staff.
•Contribute to the process of efficient and effective staff utilization.
•Promote business growth and development.
•Participate in Community events.
•During an emergency contacts manager for assignments which may be out of daily responsibilities.
•Perform other duties as assigned consistent with the skills and training and the goals and mission of Pathways.
