- Complete CMC Recertification Application Form, CE Summary Forms
- Process Applicable Payments (credit card online, check by mail)
- Submit CMC Recertification Application Form, CE Summary Forms, CE Certificates by the stipulated deadline noted in the recertification courtesy notice.

CMC-Inactive Fee: $75.00
CMC-Retired Fee: $75.00
Replacement Certificate: $15.00
Late renewal (up to 90 days late): $75.00

Late Fee (90+ days late) OR Extension Fee (must be requested in writing): $100.00
