She is also a co-host of the television talk show Aging Untold, which explores real conversations about aging, caregiving, and longevity.

Goals and Learning Objectives
Using the Fragile Years to increase participants’ understanding of how care management supports families navigating aging-related crises by providing practical strategies for navigating fragmented healthcare systems, improving care transitions, and maintaining dignity and connection during the fragile years of aging.
At the conclusion of this program, participants will be able to:
- Identify common challenges families face during aging-related care crises and explain why healthcare and long-term care systems are difficult to navigate.
- Describe the role of care management in coordinating services, advocating for older adults, and reducing caregiver stress during critical transitions of care.
- Apply practical strategies from The Fragile Years to help families stay ahead of care crises and make informed decisions aligned with their loved one’s values and wishes.
Continuing Education Information
This event has been approved for 1.5 CE contact hours through NACCM. Attendee must attend entire webinar to receive a CE certificate.
Cancellation Policy
The registration fee for webinars is non-refundable. Registrants who are not able to attend the webinar will receive audio & video files following the event. CE contact hours will not be available to those who are not able to attend the entire live event.
In the unfortunate event that a webinar needs to be canceled or rescheduled NACCM will notify the registrant by email and provide information on the new date or alternative arrangements. If the registrant is not able to attend the new date NACCM will refund the registration fee.
