Infinity Care Management is a geriatric care management agency that is seeking a PART TIME social worker (LBSW or LMSW, or other social worker status) to see geriatric / senior adult clients in their homes for one on one resource case management, assessments, and assisting activities that are living in the Greater Austin area and surrounding communities. It is mandatory that the candidate chosen for this flexible, part time position has the proven ability to be self-directed, self-managed, self-disciplined, and work effectively in an independent work environment. (client homes/their own work-from-home office)

MANDATORY Job Requirements:

Please understand that we are ONLY interested in interviewing social workers with strong home visiting experience, geriatric experience, & case mgmt./care mgmt. In addition, this person needs to have excellent computer skills & feel extremely comfortable with creating/sending PDF files, MS word, and use a reliable email account like Outlook. It is required that the candidate own a laptop or desktop computer, a printer/scanner, and a smartphone with email & texting capabilities. The chosen candidate will have to satisfactorily pass a background check and urine drug screen. In addition, there is a lot of patient & family interaction, so because of this, the candidate MUST have EXCELLENT ENGLISH speaking skills & a PROFICIENT command of the English language both orally and written. The patients & their families must be able to EASILY understand & comprehend your verbal patient teaching voice. Job Duties:

Initiate the Infinity Care Process for each new client assignment
Make an initial assessment visit to each new client and perform a multifaceted Comprehensive Geriatric Assessment
Develop a care management (CM) “Plan of Care” in conjunction with the client/family
Create a “Recommendations Report” based on the client assessment for the family members and present that to the client and/or family members
Host a case conference call with the client and/or family members & the Infinity Care Management National Director
Make follow up home visits to the client to coordinate, activate, modify, & evaluate the Plan of Care activities
Provide ongoing & PRN medication compliance checking
Provide ongoing client teaching on home safety, health risks, MD appt compliance, etc.
Coordinate & activate community & state agency resources for client’s benefit
Act as a patient advocate and liaison to physician & other ancillary health care team members
Escort clients to physician appts & other health care appts as needed
Maintain flexible schedule to meet client & family needs
Complete & submit documentation within 48 hours of home visits and report any significant concerns to the Infinity Care Management National Director ASAP
Interview, Hire, Manage, supervise, evaluate all personal home caregivers as needed by the client
Provide ongoing home assessments for safety, health, & nutritional risks and update the Plan of Care as needed
Provide counseling/family support/mediator services as needed
Provide assistance/guidance for (or interaction with) guardianship/conservatorship
Provide collaboration/assistance with or for any fiduciary needs as needed
Assistance with insurance plan, Medicaid/POA/Advanced Directives paperwork, and/or financial coordination, etc. as needed

MANDATORY Job Qualifications:

A sincere love & calling to work with & serve senior adults and a passion to assist them to age gracefully in their own homes
Current social worker license with the state of Texas (LBSW, LMSW, LCSW, or other)
Minimum of 2 years care management experience (will consider training the right candidate with at least 3 years of case management experience)
Minimum of 2 years of home visiting experience (care mgmt., hospice, home health, public health, LTC insurance assessments, health insurance, community health, etc.)
Minimum of 5 years geriatric (senior adult) experience
Minimum of 3 years computer/software experience
Current CPR card
MUST have a laptop or desktop computer & a scanner/printer
MUST have a mobile smartphone that is capable to receive emails & texts messages

HIGHLY DESIRED Optional Certifications:

CMC (Care Manager Certified)– from the National Academy of Certified Care Managers
CCM (Certified Case Manager)– from the Commission for Case Manager Certification
C-ASWCM (Certified Advanced Social Work Case Manager) – from the National Assoc of Social Workers
C-SWCM (Certified Social Work Case Manger)- from the National Assoc of Social Workers
Membership with Aging Life Care Association

Compensation:

This position pays $40 to $50/hour depending on your experience level. This hourly rate is paid out to you for ALL “Billable Hours”. Billable hours includes any time that is spent in the patient’s home, on the phone, generating reports, having case conferences, or any phone calls made to family/physicians/caregivers/ancillary health care providers/home health/insurance company, etc. Billable hours also includes any time spent emailing updates to family members or MDs, research by phone or Internet, collaborating, escorting client to MD appts/lab appts/diagnostic testing, as well as conducting assessments, etc. One way mileage & travel time is paid out and in some cases round trip mileage & travel time is paid out.
