Our Resident Care Manager serves as primary facilitator and liaison for residents of Independent Living and Enhanced Services and their family members.

Providing psychosocial and social work, and coordinates with Senior Manager of Resident Life and Wellness to meet the psychosocial needs of the residents and provides care management services for both residents and families.

Ensures efforts to protect the rights of residents in all aspects of living, decision-making and care.

– Bachelor’s degree in Social Services, Human Services,
Gerontology, Nursing, Public Health, or related field.
– Certification in Care Management preferred, and we will
support your obtaining your certification.
– Experience with Lifeplan Communities.
– Excellent communication skills and ability to mediate
resident and family concerns
– At least two years experience working with the aging
population.

Duties and and responsibilities include:

Identifies psychosocial needs of residents and develops, promotes, and implements plans/programs to address and recognize needs, such as formal support groups. Interacts personally with residents and families to establish and implement plans of care.
At the direction of Senior Manager of Resident Life and Wellness, assists in coordinating services and resident transfers to other levels of care including discussions with families and residents on the need for changing levels of care. Works closely with the Move-In Coordinator on the logistics of the move.
Serves as a member of the Admissions Committee and conducts pre-admission interviews to determine if prospective residents meet established admission criteria.
Assists new residents with adjustment and orientation to our community. Provides education, support, and outreach to each new resident on moving day and throughout their first year. Coordinates on-going New Resident Orientation to facilitate community orientation. Works closely with the Welcoming Committee to acclimate new residents to the community.
Coordinates on-going Transition Education Sessions to facilitate community wide understanding of our continuum of care, services available in each care setting, and the process for moving to a higher level of care when needed.
Provides assistance and mediation to residents and families who are experiencing significant psychosocial problems. Helps to assess short and long-term needs of the residents and coordinate services.
Assists residents in making in-house transfers within the same level of care. Coordinates with Marketing and Chief Financial Officer to determine apartment availability and financial outcomes.
Screens and monitors Resident Hired Workers to ensure policy is being followed and safe-guards are in place to determine the qualifications of individuals to perform the desired work.
Assists Senior Manager of Resident Life and Wellness with strategies and interventions to assure proper resident placement in the campus continuum and develops support systems for residents and families once a transition has been made.
Supervise the Connections Program and TR Specialist to ensure all facets of this comprehensive life-enrichment program are being delivered.
Oversees volunteer programs for residents. Coordinates with the Executive Director of the Foundation to develop our annual Social Accountability Report.
Advocating and educating residents navigating the healthcare system and necessary services.
