The Professional Care Manager provides comprehensive care management services to a variety of clients. A typical caseload is 15-20 clients which may include the frail or confused elderly, the developmentally disabled, the mentally ill, or individuals with problems of chronic physical ailments and/or disabilities. Clients may live at home or in any type of community/or institutional setting.
