Silverado is presently seeking a Life Care Manager / Geriatric Care Manager to join our At Home team.
Qualifications:
Education: Master’s Level Social Worker (MSW or LCSW) or Registered Nurse (RN, BSN, MSN).
Experiences: Several years of experience in geriatric care management, home care, long-term care or other senior-service related field.
Other: A successful track record indicating a working knowledge of clinical geriatric care issues, family dynamics and interventions, education and training, supervision, quality assurance, customer service and marketing available services. Driver’s license, insurance and willingness to drive to client locations.
Summary of Major Functions:
The Geriatric Care Manager directs the care plan and interventions designed for specific individuals and families.
This effort may include assessment, counseling, coordination of needed services, assigning and ensuring quality delivery of care-giving within home settings, assistance with placement if indicated, and communicating any additional available services to the families
This is a full-time position that is scheduled during the day.
