PRIMARY DUTIES AND RESPONSIBILITIES:
•Conducts assessments of referred clients and completes all applicable paperwork on time.
•Develops written Plans of Care for clients/families of the practice.
•Maintains client records. Completes and maintains all necessary documentation including clinical and billing documentation on a timely basis.
•Orients clients and their families/caregivers to the practice’s services and activities; arranges and monitors services as appropriate.
•Maintains regular in person, phone and written contact/interaction with client, family/caregiver, and other parties in order to coordinate services and monitor home situation.
•Communicates any changes in client status or need to the appropriate parties. Participates in team meetings as approved by team leaders/supervisors.
•Meets productivity goals as outlined by the practice.
•Provides a high level of customer services. Is available as needed by clients and their families. This may include weekends and evenings. Finds coverage within the practice when out of town or going to be unavailable to clients and their families.
•Participates in the development and implementation of a means of evaluating program effectiveness, client and caregiver satisfaction, etc. Participates in Continuous Quality Improvement processes.
•Performs other relevant duties as necessary.
•Participates in assuring compliance to all in-house, local, state and federal regulations.
MINIMUM QUALIFICATIONS:
A Master’s Degree in a field related to care management, including registered nurses and nurse practitioners, and a minimum of 2 years of paid care management experience within the past 10 years.
OTHER REQUIREMENTS:
•Experience in geriatrics and/or care management required.
•Previous individual work with the elderly, preferred
•Care Management Certification (CCM or CMC) or a Certified Nurse Practitioner (NP) a plus
•Experience and demonstrated competence and comfort working with families in crisis.
•Excellent interpersonal communication and organizational skills
•Ability to communicate successfully with clients, staff members and community resources across the social services and medical spectrum
•Able to work collaboratively with a team as well as independently
•Detail oriented and able to communicate clearly through oral and written means
•Valid Ohio driver’s license and proof of current automobile insurance
•Position requires transportation to make visits to places on the client’s behalf
