New Position: Geriatric Care Manager

Health and Comfort Home Care is expanding its services and is looking for a Full-Time Geriatric Care Manager for its newest office in Red Bank, New Jersey. The ideal candidate will have clinical and business experience in building relationships with the following:
• Discharge Planners
• Case Managers
• Physicians
• Nurses
• Hospitals
• Rehabilitation Facilities
• Skilled Nursing Facilities
• Long Term Care and Insurance companies
• Geriatric Care Managers
• Elder Care Attorneys
Job Responsibilities
• Ongoing Management and Oversight of Care- ranging from a minimum of an initial assessment for a consult for future care to coordination of all of the patient’s medical and social needs
• Care Coordination- identify appropriate elder care coordination as needed for: rehabilitation programs, medical facilities, physicians, elder care attorneys, assisted living, social activities, adult day programs, long term care as needed. This service may also encompass assuring smooth transition with moves to or from an assisted living facility, hospital or nursing home
• Aging-in-Place Solutions- Assist clients whenever possible by identifying solutions for them to stay at home rather than moving to a long-term care facility through the use of technology, home modification, home care, and/or durable medical equipment solutions
• Comprehensive Patient Assessments- for senior and disabled adults’ well-being to determine care needs and develop a plan of care
• Liaison Service for Families at A Distance- or otherwise unavailable-by communicating with providers as well as entitlement coordination, scheduling and arranging appointments for patients, checking in on family member with regularly scheduled visits and ongoing communication with all family members. May also include arranging/managing transportation to medical appointments, monthly bill payments, weekly medication preparation, hospital admissions, nursing home placement, hospice and end of life hospice counseling
• Home Safety Evaluations- when a client or family member wants to evaluate and maximize the safety of a family member at home. This includes making recommendations and identifying resources regarding home safety solutions and renovations
• Long-Term Care and Housing Planning – This encompasses working closely with physicians, elder law attorneys, bank officers, home health care agencies to ensure aging services are carefully planned and asset preservation, legal protection, and overall health are maximized. May require review of financial, legal, medical and family concerns and refer family members to specialists in their area
• Community Resources Coordination – to identify resources that may include religious, government, Medicare, Medicaid, Managed Care non-profit and private programs.
Education Requirements
• MSW, LCSW
• Bachelor’s Degree in Nursing
Required Experience
Must have a successful track record with a minimum of two years experience working with private clients in the following disciplines: geriatric, rehabilitation, and mentally ill population and familiar with:
• Long Term Care Options
• Senior Housing
• Long Term Insurance
• Hospice
• Home Care
