Alliance Health is a leading edge group of 8 provider offices focused on patient centered care. As we drive our CPC+ program and other care management programs, we are specializing in treating the whole patient, connecting care from their family care provider, to cardiology, and even to rheumatology and many others. This model includes the Care Navigator who will be responsible for the care management of patients in a primary care office. He/she will work in collaboration with primary care providers and care team members to develop and monitor care plans for patients, including facilitating the proper resources to address patients’ needs.

Qualification for the successful Care Navigator include but are not limited to:
•Licensed Masters in Social Work with at least 2 years of experience in related field
•At least 2 years experience working on care management
•Understand care management processes including care planning
•Experience providing outreach, disease management and/or self-management services to patients and families
•Knowledge of quality metrics for chronic conditions and preventive services
•Ability to communicate with patients, primary care physicians and the care team
•Comfort in working with brief, solution-focused interventions in a time-limited setting
•Flexibility in a fast-paced, primary care environment
•Strong assessment, triage, diagnostic and therapeutic skills
