OVERVIEW: Care Management is a collaborative process that includes assessing physical, social and environmental factors that affect the lives of iCircle Care members. The Care Management Supervisor is responsible for the overall direction, daily management and oversight, monitoring, and staff education of a team of Care Managers who implement an appropriate quality and cost effective plan that utilizes all resources available to meet those needs. The supervisor oversees and coordinates the activities of the assigned care management team.

RESPONSIBILITIES:

•Provide support, supervision, training, oversight and quality review of a care management team.

•Participate in hiring of new care managers, including interviewing, reference checks, etc.

•Meet regularly with team to review member’s plans of care and needs on a consistent basis including member’s response to interventions to ensure delivery of quality, cost effective care.

•Assist Care Managers in developing plans of care that include local providers. Develops relationships with network providers to ensure quality of care of iCircle members.

•Assist Care Managers in resolving member issues.

•Assist in training new Care Managers.

•Approve services based on iCircle benefits.

•Review and authorize request for services according to workflows and timeframes.

•Maintain clear, effective communication with member continually, document all contacts.

•Educate Care Managers, members and caregivers on covered services and benefits.

•Evaluate available clinical data and notes, including assessments.

•Collaborate with interdisciplinary team and other iCircle departments as appropriate.

•Assist department in meeting performance benchmarks.

•Participate in hospital or Skilled Nursing Facility discharge meetings as needed.

•Document all interventions and interactions in electronic record.

•Participate in monitoring statistics, trends and reports to implement process improvement strategies.

•Maintain member privacy, confidentiality, health, and safety.

•Adhere to ethical, legal, accreditation, certification, and regulatory standards and guidelines.

•Demonstrate cultural competence by being respectful of and responsive to the health beliefs, practices, cultural and language needs of the member and his/her support system.

•Participate in Quality Assurance and Improvement activities.

•Ensure iCircle policies and procedures are followed and assists with the development of policy and procedure and standard operating procedures as needed.

•May carry caseload depending on agency needs. Provide on-call coverage.

•Performs all other duties as assigned by supervisor.

ESSENTIAL ACADEMIC AND EXPERIENCE REQUIREMENTS:

•New York State Licensed Registered Nurse, Bachelor’s degree preferred OR Licensed Master’s Social Worker

•Minimum seven years in healthcare field, with 2 years as a care manager in managed care environment or community health

•Knowledge of Medicaid and Medicare regulations required

•Knowledge of community based providers

•Previous supervision experience a plus

•Reliable transportation (NYS Drivers’ Licensed required)

•Ability to work collaboratively and effectively with the interdisciplinary team members, iCircle members, families and providers

•Ability to work independently and motivate others

•Ability to communicate effectively, both orally and in writing

•Maintain all required certifications/training by State regulations and iCircle policy

•Excellent verbal and written communication skills, expressing self in a clear, concise and professional manner

•Bilingual a plus

•High attention to detail

•Experience utilizing computer software including Microsoft Word and clinical applications

•Reliable transportation (NYS Drivers’ Licensed required).

•Must be at least 18 years of age

All experience and education requirements, except when required by federal, state, or local laws or requirements, may be waived at the discretion of management with the approval of the Program Vice President in collaboration with Human Resources.

•Attend mandatory Education and Training modules as scheduled, obtains, and maintains required license and certifications.

•Act as a professional representative of iCircle Services in regards to appearance, behavior, temperament, communication, language, and dress.
