The position requires a licensed/certified health and human service specialist responsible for client(s) assessment, care coordination, advocacy, communication and other related care management responsibilities. Client base is predominately older. Inherent in the position is working in conjunction with client and/or family, physician, and ancillary health providers with the goal of promoting independence utilizing appropriate high quality and cost-effective services. The position requires a team-minded individual who will collaborate with the Owner and other designated Inspire Care team members to attain client centered care management goals. A proactive individual who is positive, a good listener, outgoing, with proven communication skills (written and verbal) is a must.
