Certification Renewal Policy

Certification from the National Academy of Certified Care Managers (NACCM) must be renewed every three (3) years. The purpose of Certification renewal is to assure that the Care Manager Certified (CMC) continues to practice as a Care Manager and has continued to expand his/her knowledge base of care management. Certification renewal is valid for a period of three (3) years.

The CMC will receive the renewal application about 30 days prior to expirations. It is the CMC's responsibility to complete and submit the application for Certification renewal by the date listed on the renewal notification letter.

If certification lapses for more than six (6) months, the CMC must meet all requirements for initial certification and take the certification examination.

Renewal Criteria

1. Provide a minimum of 1500 hours of care management service as a care manager, care manager supervisor or administrator of a care management program during the three (3) year certification period.

AND

2. Earn 15 contact hours of continuing education per year for a total of 45 hours over the three (3) consecutive years of certification;

AND

3. Provide written verification of employment, supervision and continuing education on the forms provided by NACCM

The CMC must submit the Renewal Application, Employment Verification Form, Continuing Education Summary Forms and application fees by the date listed on the renewal notification letter (usually the certification expiration date). Applications received after the expiration dates are subject to a $50.00 late fee. Upon approval of the application, a new certificate is mailed to the CMC.

If the CMC cannot meet the above criteria, s/he may apply to take the written examination. The CMC will need to purchase a candidate handbook ($25), complete all application forms and submit the application to PES with the $225 application fee.

If certification lapses for more than six (6) months, the CMC must apply and meet requirements for initial certification and sit for the CMC examination.
