Application Process
A Candidate Handbook and application form
may be obtained from NACCM's central office for $25.00. Payment
should be in the form of a check or money order made payable to
the National Academy of Certified Care Managers.
In order to be scheduled to take the Certification Examination
for Care Managers, an application must be completed. Only original
application forms (no photocopies) will be accepted.
The application form, work and education verification forms and fee must be mailed to National Academy of Certified Care Managers (NACCM). The application fee must be made payable to NACCM.
NACCM will notify candidates if a submitted application form
is incorrect, incomplete, or contains an incorrect fee. An additional
fee of $60.00 is to accompany the resubmission of the required information.
If the candidate does not respond to NACCM ’s notice by submitting
the required information within 20 days, the candidate will be considered
to have withdrawn from the examination, and the application fee,
minus a $75.00 processing fee, will be refunded.
