Eligibility

In order to be eligible to take the Certification Examination, candidates must meet one of the following three criteria:

Education

Supervised
Care Management Experience

Additional
Direct Client Contact

CRITERIA I    
Masters Degree in a field related to care management
And>
> 2 years of paid, full time, supervised care management experience subsequent to obtaining education requirement, and within last 10 years
 
CRITERIA II
Bachelors Degree in a field related to care management
And>
> 2 years of paid, full time, supervised care management experience
And>
2 additional years of paid, full time direct experience with clients in fields such as social work, nursing, mental health/counseling, or care management subsequent to obtaining education requirement, and within last 10 years
CRITERIA III
Associate’s Degree in a field related to care management
- or -
RN Diploma
-or-
a bachelors or higher degree in a non- human services field
And>
> 2 years of paid, full time, supervised care management experience
And>
4 additional years of paid, full time direct experience with clients in fields such as social work, nursing, mental health/counseling, or care management subsequent to obtaining education requirement, and within last 10 years

*Fields related to care management may include:  social work, counseling, nursing, psychology, gerontology, rehabilitation, public health, or human services.

Care management experience must include all of the following tasks:
A face-to-face comprehensive assessment; problem identification; goal setting; care plan development, implementation and monitoring; reassessment; and quality evaluation.  Care management is done with individuals, not communities.  Care managers follow clients across settings and work with care managers of specific settings.  The comprehensive assessment must be face to face and include all of the following domains: health, function, social, emotional, cognition, environment, spirituality, support system, and finances. 

Care management and client experience must be subsequent to obtaining education requirement and within 10 years of the date of application.

Supervision/Consultation is defined as individual, group or peer review of performance, use of clinical skills and core care manager functions, record review, peer consultation, case review and/or case conference in the amount of 50hrs/yr for two years.
