General Summary:
Reports to the company owners and is responsible for the care management/care delivery and oversight of the CNA staff caring for the day to day needs of the Care management client. The Care Manager participates in efforts to expand and develop business opportunities.
Principal Duties and Responsibilities:
The Care Manager will conduct comprehensive assessments of the client’s physical, functional and emotional state as well as the client’s living environment. Assess the client’s perception of need and expectations for service. Utilizes appropriate screening tools as needed. Maintains awareness of resources needed for the geriatric population.
Develops written care plan to comprehensively address identified needs that is consistent with Ella Stewart Care Management (ESCM) standards of care and patient wishes.
Reviews plan of care with treatment team and client.
Implements established care plan by coordination of services and thorough education to home care staff. Assists in the development of education plans for caregiver staff.
Evaluates and modifies care plan to address changes in client condition in conjunction with the treatment team.
The Care Manager will assume a proactive role in marketing ESCM services to the community. She/he will also advise marketing staff of prospective clients and contacts.
The Care Manager will keep abreast of research and developments in the care of geriatric patients to maintain ESCM leading edge position in the market place.
Minimum Knowledge, Skills, and Experience Required:
• BSRN licensed in the State of Maryland (Masters
preferred)
• Five years of geriatric clinical experience
• Experience in home health care for seniors
• Highly effective verbal and written communicator with
the clients, families, and the caregiving staff
• Organized and detail-oriented
• Competent Computer skills, including Word and Excel
